This approach helps in reducing prolonged hospital stays and prevents readmission, as service users do not remain in inpatient care longer than necessary. We provide a comprehensive range of services and support adults in progressing along a planned pathway to a more independent lifestyle.

Nicola, a qualified mental health nurse, possesses a wealth of experience in supporting both adults and adolescents across diverse environments, including Psychiatric Intensive Care Units (PICUs), Child and Adolescent Mental Health Services (CAMHS), and community-based teams. In addition to her nursing expertise, Nicola is also skilled as a health and wellbeing therapist. She applies her extensive knowledge in this field to uniquely enhance the comprehensive holistic wellbeing of those she serves and nurtures.
Natalie Prudent
Care Coordinator
Natalie, our care coordinator and nominated individual has over twenty years experience in healthcare with her most recent being in the Short-Term Supported Mobile Intervention (STSMI) services. Her background knowledge and working experience enables us as a team to respond, train, and facilitate staff to work in a fast-paced environment while ensuring the highest quality of care, promoting independence.
Our Wider Team
We have experience providing services in fast response environments, including supporting fast hospital discharge, short-notice package commencement, and preventing hospital admission for those with mental health difficulties, learning disability and/or autism, and those who need physical health support. We also support mothers & babies settle into a safe and nurturing environment.
Our staff are trained to respond quickly to urgent and/or challenging situations, and we have established protocols for efficient communication and coordination with healthcare facilities, social services, and other stakeholders.
We use a strengths-based approach in supporting service users,which involves focusing on the individual’s inherent strengths, resources, and capabilities, rather than just their problems or deficits.
Our current training programs are tailored to promoting independence and reablement work, ensuring that our care staff are well-prepared to deliver the required services to the highest standards.
