Navigate Supported Living is a nurse led service. We are able to offer accommodation across the Midlands and Staffordshire for vulnerable adults with a range of support needs.

Admissions Criteria

  • Adults aged 18 and over.
  • Emergency Placements – Young People 16+
  • Individuals who have a range of low – complex support needs including Mental health, ASD, Neurocognitive disorders, LD and ABI.
  • Adults with a forensic/significant risk history requiring a more robust community-based environment.
  • People who are informal, subject to DoLs, on conditional discharges, CTO’s or on licence by the Parole Board.
  • Adults aged 18 and over.

Our range of support

We assess across a variety of factors including risk to themselves and others, challenging behaviours, and the relational environment between users of the service. We anticipate individuals will present with support needs that may require higher support and can accept under the following therapeutic milieu

  • Level 1: The individual will have some independent living skills but still needs ongoing support 24 hours a day with minimum needs of a few or occasional hours of 1:1 in a 24-hour period.
  • Level 2: Clients will be identified as having less independent living skills and hence require a higher input of support from staff with several hours of 1:1 support in a 24-hour period.
  • Level 3: Clients will require support either as a minimum of 1:1 24 hours a day or 2:1 support 24 hours a day.

We can adapt the service according to assessed needs so if required we can create a bespoke package of care including the creation of a single person service.

The service follows CQC regulations and delivers care using the PBS framework. There is ongoing supervision and quality assurance of the service by a wider management team.

  • Specialised Supported Living:  Focused care for adults with learning difficulties, autism, and varying physical and mental health needs. Our strengths-based approach highlights and develops each individual’s abilities and interests.

  • Therapeutic Living Spaces: Environments accommodating up to three residents ensure the highest level of support and individual attention, including specialised care for physical and mental health, & learning disability needs.
  • Outcome-Focused Support: Recovery and outcome-oriented, leveraging each resident’s strengths in both physical and mental health aspects.
  • Flexible Support Ratios: Offering one-to-one or two-to-one support, tailored to meet individual physical, mental, and emotional needs.
  • Targeted Services: Access to therapeutic interventions for both physical and mental health, provided by selected experts, aligning with our strengths-based philosophy.
